Panhandle Home Health nurses plan care for a patient
Panhandle Health District's Home Health division provides a service or combination of services in a patient's home under a doctor's direction. PHD's qualified staff plans, coordinates and provides care tailored to the patient's needs.
Services include:
- Skilled Nursing
- Provided by a Registered Nurse (RN) or a Licensed Practical Nurse (LPN) with training and experience in providing care in the home. The nurse frequently updates the patient's plan of care with his or her doctor. Skilled nursing services include:
- Evaluation of patient needs
- Performance of skilled nursing procedures
- Education of the patient, family and caregivers on disease processes, self-care techniques and prevention strategies
- Coordination of patient care and services with the patient's doctor and other members of the health care team
- Provided by a Registered Nurse (RN) or a Licensed Practical Nurse (LPN) with training and experience in providing care in the home. The nurse frequently updates the patient's plan of care with his or her doctor. Skilled nursing services include:

Home Health Aides
- Provide bathing assistance for patients receiving skilled nursing services and/or physical, occupational or speech therapy under a doctor's order.
Emergency Preparedness Plan
- Panhandle Home Health has an emergency plan to continue necessary patient services during a natural disaster, bad weather or other emergencies. When staff safety is at risk, Home Health will contact patients by telephone. Every effort will be made to ensure that patient medical needs are met.
