March 6, 2006 Panhandle Health District Calls for "Health Heroes" Nominations

Panhandle Health District is calling for nominations from the community for Health Heroes - individuals or business/organizations who worked to protect, promote, and enhance the health of North Idaho citizens during 2005. Award winners will be honored during Public Health Week, April 1-9. Nominations are due by Friday, March 24 and will be judged by PHD's Public Health Week committee. Nominees must be residents of or do business in Benewah, Bonner, Boundary, Kootenai or Shoshone counties. Examples of activities for which individuals or groups may be nominated include, but are not limited to, the following categories:
Nominations are being accepted in adult, youth, and business/organization categories. The "Adult Hero" award will honor an adult individual or group in the community who has made a long-term commitment to improve the health of citizens in North Idaho. Nominees may work in any field and do not have to be a part of an organization that works in health care. The "Youth Hero" award recognizes a youth (18 years or younger) or youth group in North Idaho who has worked to improve the health of the community. The "Business/Organization Hero" award recognizes a business or organization that worked to educate the public about healthy living in 2005. Nominations must include the nominee's name, age, gender, address and phone number, as well as the name, address and phone number of the person who is submitting the nomination. Nominations must also include a brief description about why the nominee deserves to be recognized as a Health Hero. Nomination forms are available at all PHD offices and on the PHD website at www.phd1.idaho.com. Nominations also may be submitted by email to scuff@phd1.state.id.us.
